Photocopy of all medical insurance cards (OHIP, Blue Cross, etc.).
Names and phone numbers of relatives.
Names and phone numbers of family doctor and/or any specialists.
Copy of birth certificate.
A list of all prescription and non-prescription drugs.
A list of all electric appliances brought into facility.
Non-skid slippers for indoor use.
Any walking aids you have have.
Furniture, if occupying any unfurnished room (with accompanying insurance).
Your own personal care items.
Towels and linens will be supplied by the facility.
Valuables can be locked up in the office.
- All medications should be prescribed by a physician, dispensed by a pharmacy, and supervised by residence staff.
Fee Structure:
Shared Room: $50.00 per day
Private Room: $65.00 per day
Large Private Room: $75.00 per day
